About Dr. Al Aly
I grew up in Southern California and completed a bachelor’s degree in psycho- biology at UCLA. Subsequently I received my medical degree from Georgetown School of Medicine in Washington DC and then spent two years in general surgery training at UCLA with another two years of research at the same institution. I then did another one-year research fellowship and then completed an ENT residency at Vanderbilt University. I then moved back to the Southern California area where I completed a fellowship in facial plastic surgery, mostly concentrated on rhinoplasty or nasal reshaping surgery, at the University of California Irvine. Because I trained originally in ENT and received special training in rhinoplasty, I have special interest and expertise in rhinoplasty.
At this stage of my career I thought that I would end up practicing facial plastic surgery so I joined a private practice in the Beverly Hills area for 2 years. I realized during that time that I have true passion for the entire specialty of plastic surgery. Thus I decided to go back and retrain in all of plastic surgery by completing a plastic and reconstructive surgery residency at the University of Miami Jackson Memorial.
After completing my second residency, I started my working career in plastic surgery at the University of Iowa which was a life and career altering event. The University of Iowa happens to be the birthplace of bariatric surgery, which is surgery that is performed by general surgeons to reduce obese patients’ weight. At the time I joined the faculty of plastic surgery at the University of Iowa, bariatric surgery was fairly limited to the Midwest, mostly Iowa and Minnesota. This led to a situation where massive weight loss patients, which at the time was an unusual group of patients to encounter in a plastic surgeon’s career, started to frequent my practice. These patients were seeking a return of their bodies back to normal. In attempting to help them I quickly realized that the traditional procedures that plastic surgeons learned and performed were inadequate and led to less than satisfactory results and patient disappointment. This pushed me to create and develop new plastic surgery procedures specifically designed to treat the complex problems that massive weight loss patients presented with. Thus I pioneered the field of “Body Contouring after Massive Weight Loss” during this period, also at the University of Iowa.
Dr. Aly’s Publications
Around the year 2000, bariatric surgery techniques were being developed that made weight loss surgery safer, more successful and more easily tolerated by a large number of patients. This led to a very steep rise in the number of patients losing weight after undergoing bariatric surgery. As noted above prior to 2000, these patients were mostly limited to the Midwest, but after these bariatric surgery developments, literally hundreds of thousands of patients started to undergo weight loss surgery across the US, which led to a very large number of patients who needed to eliminate their excess skin and fat. Plastic surgeons were suddenly faced with a huge number of patients that needed body contouring after massive weight loss but they did not have the expertise. To alleviate this problem, I became involved with the education of plastic surgeons by lecturing, doing surgical demonstrations and publishing the first ever textbook dedicated to the subject entitled “Body Contouring after Massive Weight Loss” in 2006. This book has since been recognized by many plastic surgeons nationally and internationally as the “Bible” of this kind of surgery. Over the last 25 years I have been fortunate enough to either lecture and/or perform surgical demonstrations in over 36 countries to educate plastic surgeons on how to deal with this patient population, which is ever growing, both nationally but internationally.
Patients Love Dr. Aly
It was extremely easy to contact Dr. Aly’s office to get my initial consultation, pre-op lab work requested by Dr. Aly as well as my procedure scheduled. I was explained in great detail what to expect and the limitations of the procedure. The payment process was very easy and I had all of my questions answered promptly.
It was easy to get all post op question answered by Dr. Aly or his staff–very friendly, respectful and accessible.
Based on my contributions outlined above, to what is now a subspecialty of plastic surgery, I was bestowed the “Lockwood Award” which is given to plastic surgeons who have made major contributions to body contouring surgery. This award has only been conferred 6 times.
My career has taken me to many places in my life. In the 13 years I spent in Iowa, I got married to a wonderful woman, had 2 great children and grew immensely in my chosen profession. I was eventually recruited to the University of California Irvine where as a full professor of plastic surgery I became the vice-chair of the division of plastic surgery. While there I helped plastic surgery transition from a division to a department, at least partially due to my national and international prominence. Four years later I was recruited to help open a brand-new hospital for the Cleveland Clinic in Abu Dhabi and to start the plastic surgery section there. I spent 6 years there, was appointed the section head of plastic surgery, and built a very busy practice cementing my expertise in massive weight loss body contouring and rhinoplasty. Lastly I joined the faculty at University of Texas Southwestern plastic surgery department, in Dallas Texas, again as a full professor in November of 2020. This final position brings me back to my academic roots allowing me to further grow my areas of expertise as well as being intimately involved with the education of the next generation of great plastic surgeons and to pursue research on a number of subjects including understanding beauty and attractiveness. I am very proud to have joined this department which is recognized as one of the premier plastic surgery training programs in the world.
On a personal and social level my wife and I are very involved with our children. My son aspires to be an engineer and has been accepted at an excellent engineering University. He may, or may not, pursue a career in medicine after that. My daughter is in high school and we endeavor to give her the best opportunity to realize her potential.
Passions are the key to life and I am fortunate to have a few. My life’s passion has been, and still is, plastic surgery. It is a specialty like no other in medicine which continuously challenges one’s intellectual capabilities. I feel lucky in life because I very much enjoy what I do for a living. In general, I appreciate and delight in sports having participated in a wide variety in my life, especially soccer. I have played soccer at a fairly high level (college and semi-pro) in my youth and played it up to a very short time ago. Currently I stay in shape through swimming and light weightlifting. I am an avid reader with a wide range of subjects that interests me. I love to learn about almost anything I do not know much about.
Last but not least I have been fortunate enough in my career to have an opportunity to travel extensively, which I love. Although the Covid pandemic has slowed down everyone’s travel, including mine, it is my hope that once things open up I will be able to go back to traveling to many places around the world.

My decisions in caring for my patients is based on “if I would not do it on my own family member, I will not do it on my patient.”
